Review the list of Primary Care Physicians who are accepting new patients and choose one who is best for you.
Search the physician directory to learn more about each physician. You may also request a copy of our Medical and Services Directory. Send Email.
Call our Find-A-Physician referral line at (541) 757-3757 in Corvallis or (541) 791-2565 in Albany, or email us. Send Email.
Contact the Member Services Department of your primary insurance carrier. The telephone number is listed on your insurance ID card or you may find it in our link to Insurance Providers.
Inform your insurance carrier of your physician choice and ask them to change your records. You should receive a new identification card confirming your selection. |
