Responsibility for Account
- Individuals are responsible for their own account regardless of insurance coverage.
- With regard to families, the account holder is typically the head of the household who may not necessarily be the individual with primary insurance coverage.
- Parents or other assigned legal guardian(s) with whom children reside are considered the account guarantor and as such are responsible for payment for minor children. The Corvallis Clinic is not responsible for managing divorce decrees or other legal matters pertaining to payment responsibility.
- Children age 18 and older are considered adults and will be responsible for their own account.
- If a parent wishes to maintain account-holder status, arrangements can be made through the Billing and Insurance Department.
- Claims payment and/or non-payment disputes with insurance companies are the responsibility of the patient.
Co-Payments
Most managed care plans include a co-payment that is due and payable at the time of service. Some co-payments are based on a percentage of charges.
