When you arrive for your appointment, please check-in at the front desk. Your insurance coverage and other demographic information will be verified and updated. This includes your address, phone numbers, insurance information and any other special billing instructions.

Patients may pre-register prior to their appointment by calling the Billing & Insurance Department at (541) 754-1279. You simply provide our representative with any updates to insurance and demographic information.

New Patients
New patients are asked to complete a registration form prior to their first visit. It simply provides The Clinic with insurance and demographic information. The patient's signature will serve as an authorization for release of medical information as well as agreement to our financial policy. This information will be updated annually or whenever a change occurs regarding the patient's registration information.

Identification Requirements
Patients must bring to each visit one form of picture identification (such as a driver's license) as well as updated proof of insurance coverage. If identification is missing or if a patient's account is not in good standing, the patient may be asked to pre-pay for services or their appointment may be rescheduled for a future date.
