The employee is required to complete the questionnaire and return to Dr William Ferguson or Dr. Christopher Swan for consideration of respirator medical clearance. Clearance can usually be granted by the physician reviewing the questionnaire alone. If any high risk factors are identified by the physician, a physical examination will be requested.
