Message and consent Macro
To be sent to the patient (when the patient initiates a portal message with a treatment request) by whom ever gets the message first (CMA, MOS):
You have requested treatment through a portal message. Prior to passing this information along to your provider, I have to discuss some details for which I need your consent in order to proceed.
1. Any advice provided will be based upon the information shared in the message, and will unfortunately be limited by the lack of direct contact. We will do our best with what is available to us, so it is important for you to give as much information as possible in the portal message. If there are more issues that cannot be covered with the portal communication, we may have to ask you to come in for a direct face-to-face visit at a later date.
2. All information you provide in the portal message will be private and confidential, and documented only within your personal medical record.
3. Your insurance will be billed for this visit. As a result, you might be subject to a copay, depending upon your insurance Your insurance may also deny the charge, and you will be responsible for it . If there is a problem with insurance and billing, The Corvallis Clinic billing department will be happy to work with you on that.
4. If you would prefer a face to face visit, I will be happy to ask our front desk to call you and schedule an in person or video conferencing visit.
5. If you believe a portal message remains the best option for your treatment right now, please message back and let me know – please add any additional or missing details you think may be helpful – and I will ensure it gets directed to your provider.
