Payment Information
Accepted Payment Types: Cash, check, and card. Payment is due at the time of service for applicable patient responsibility amounts, including co-pays, deductibles, or self-pay services.
Accepted Payers: The clinic accepts insurance plans with which it maintains active contractual agreements, including select Medicare, Medicaid, and commercial insurance plans. Acceptance may be limited based on plan type or service line. Not all plans within these categories are accepted. Patients should verify participation of their specific plan prior to receiving services.

We do not provide emergency department or specialty services. Availability of services may vary based on patient volume and provider availability. For life-threatening emergencies, call 911 or go to your nearest emergency room.
