- All adults over age 65
- Adults under 65 who have gone through menopause
- Breast cancer genetic test counseling (BRCA) for those at higher risk
- Breast cancer mammography screenings:
- Every 2 years for adults age 50+
- As recommended for ages 40–49 or those at high risk
- Breast cancer chemoprevention counseling (for those at higher risk)
- Cervical cancer screening
- Cholesterol screening (for adults of certain ages or higher risk)
- Colorectal cancer screening (adults age 45 to 75)
- Depression screening
- Diabetes (Type 2) screening (for adults age 40–70 who are overweight or obese)
- Diet counseling (for adults at higher risk of chronic disease)
- Domestic and interpersonal violence screening and counseling
- Falls prevention (for adults 65+ in community settings):
- Includes physical therapy and vitamin D supplementation
- Folic acid supplements (for those who may become pregnant)
- Hepatitis B screening (for those at high risk)
- Hepatitis C screening (adults age 18 to 79)
- HIV screening:
- For everyone age 15 to 65
- Other ages if at increased risk
- PrEP (HIV prevention medication) for those at high risk
- Lung cancer screening (adults 50–80 with heavy smoking history)
- Obesity screening and counseling
- Statin preventive medication (adults 40–75 with high risk)
- Tobacco use screening and cessation support
