The Corvallis Clinic accepts only established patients with Original Medicare. No new Original Medicare patients will be accepted.

A “new patient” is defined as someone who has not been seen by a provider at The Corvallis Clinic within the past three years.

Providence Medicare Advantage (Exclusions Apply)

Providence Medicare Advantage offers HMO plans only.

Effective January 1, 2026, The Corvallis Clinic will no longer accept new patients with Providence Medicare Advantage plans, nor established patients who have newly enrolled in Providence Medicare Advantage.

Regence Medicare Advantage (Exclusions Apply)

Only Accepted Network:

The Corvallis Clinic accepts established patients with IHN coverage and accepts new patients for pediatric-aged patients and specialty only.

A “new patient” is defined as someone who has not been seen by a provider at The Corvallis Clinic within the past three years.

Oregon Medicaid / Oregon Health Plan (Exclusions Apply)

The Corvallis Clinic accepts established patients only with Oregon Health Plan (Medicaid Open Card). No new Oregon Health Plan (Medicaid Open Card) patients will be accepted.

A “new patient” is defined as someone who has not been seen by a provider at The Corvallis Clinic within the past three years.
